Authorities are currently probing the factors behind the lethal Pennsylvania steel factory blast that claimed the lives of 2 individuals.

In the small town of Clairton, Pennsylvania, a tragic incident occurred at the U.S. Steel plant on Monday. The explosion sent black smoke billowing into the midday sky, causing concern among local residents and raising questions about the plant's safety record.

The plant, a massive industrial facility along the Monongahela River, is one of four major U.S. Steel plants in Pennsylvania. It converts coal into coke, a key component in the steelmaking process.

The cause of the explosion has not been isolated yet, but preliminary investigations suggest a failure of a gas valve during preparation for planned maintenance. Pressure built up inside the valve, which then failed, releasing coke oven gas into the area. An ignition source ignited this gas, causing the explosion [1][4].

The explosion started a fire at the plant. Five people were hospitalized in critical but stable condition on Monday night. Five others were treated and released. One of the victims has been identified as Timothy Quinn, 39 [2].

The Clairton Coke Works plant has a history of maintenance and safety issues, with previous accidents including explosions and fatalities. In 2010, another explosion injured 19 people (14 employees and 6 contractors). U.S. Steel and a subcontractor were fined $175,000 for safety violations by the Occupational Safety and Health Administration following the 2010 incident [3].

U.S. Steel's chief manufacturing officer, Scott Buckiso, is working with authorities to determine what happened. The U.S. Chemical Safety and Hazard Investigation Board is also investigating the incident [1][4].

Initially, the Allegheny County Health Department advised residents within 1 mile (1.6 kilometers) of the plant to remain indoors and close all windows and doors due to air quality concerns. However, the advisory was later lifted [5].

This is not the first incident of its kind at the Clairton plant. A maintenance worker was killed in a blast at the plant in September 2009, and the last death at the plant occurred in 2014 [6]. In February, a problem with a battery at the plant led to a "buildup of combustible material" that ignited, causing an audible "boom," but no one was seriously injured [7].

The plant has approximately 1,400 workers. U.S. Steel is cooperating with authorities to ensure a thorough investigation and to prevent such incidents in the future.
